Authorisation process
Applications for all courses conducted by FESA or by Emergency Management Australia should be submitted on an official training nomination form. There is a preprinted training nomination form for each course. A copy of the form can be obtained by contacting the Administration and Support Officer, Strategic Policy and Executive Services on (08) 9323 9336 or by emailing: Tanya.Stokes@fesa.wa.gov.au
The training nomination form has been designed to provide all the essential information required by those conducting the activity. All sections of the training nomination form must be completed in legible writing. As the nomination forms are frequently transmitted by facsimile, clarity of writing is most important.
Applicants must complete a training nomination form and have it approved by their supervisor or manger in the organisation which they will represent. Applicants representing a volunteer organisation must obtain clearance from their employer and their volunteer organisation. The approved training nomination form should be forwarded through the organisation's training representative where applicable.
Organisations which require training nominations to be endorsed by their training representative are the WA Police Service, Department for Child Protection, Department for Communities, Department of Health and FESA Operational Services. The representative will forward your authorised form to FESA.
Training nomination forms need to be received by FESA by the course closure date to allow time for selection of successful nominees.
